This project seeks to implement, evaluate, and disseminate low-cost/light touch interventions designed to increase the number of care partners with shared access to patient portals. This 36-month project funded by The John A. Hartford Foundation involves a partnership between OpenNotes at Beth Israel Deaconess Medical Center, the Roger C. Lipitz Center for Integrated Health Care, and leaders from 3 geographically diverse health systems: Providence, University of Rochester Medical Center, and University of Utah Health.
